Services offered are for individuals, couples/marriages, family, and small groups. All parties receiving therapy services must be 18 years of age or older.

You’ll get notifications when there are documents to complete or messages from me. By acknowledging this, you agree to check the portal and respond in a timely manner so we can stay on top of scheduling, billing, and any needed forms.

ACTION REQUIRED: This is only a prescreening form. In order to confirm your appointment request, you must: 1. Return to the portal to complete ALL intake forms. You will receive an email with a link to complete the forms. 2. Acknowledge that you understand and will complete the intake forms within 2 days after you receive them.

YES, I will return to the portal within the next 2 days to complete the intake forms and upload the completed insurance verification form (if applicable). If I do not complete the forms, I acknowledge that my appointment request will be declined and I will need to submit a new request after completing forms.
